Duplicate listing in My Sites

Description
This site, since the time of creation, my "Sandbox Fun" site shows up twice in my My Sites listing in the drop-down in the upper-right. The site is at https://sandboxfun.commons.gc.cuny.edu/. I can see that the menu links go to the same site, since when I make a change on one it is reflected on the other. I'm attaching a screenshot showing how it appears in my menu. Please let me know if there's other info I can provide.

There are two different sandboxfun.commons sites (3999, 4000). They were created at the same time 2018-06-19 06:59:57. This suggests it was due to a glitch in the system. I recall this happening one other time, maybe around mid-2018, but I can't find record of it; if I recall correctly, it was some specific bug that I then fixed. As such, I don't think we need a ton of investigation into root causes. But I did go ahead and delete 4000, since 3999 seems to be the one that was actually loaded at sandboxfun.commons. Let me know if you see any problems.
