Inconsistent email notifications from gravity forms

Description

Last week, I noticed that there was a new form entry for which I never received an email notification. This week, I tested it by submitted a "fake" form entry, and the notification worked. However, if I go to the entry page and send a notification from there, it doesn't work. I checked the gravity form logs, and it looks like the emails are being submitted to SMTP. But, the notifications work sometimes and don't work other times. This can be problematic since students don't typically tell me that they submit forms, they just submit them and await my response. I checked the gravity forms system status and found the following errors:

- Version: 2.5.9.3 ✘ New version 2.5.10 available.

I checked the gravity form logs, and it looks like the emails are being submitted to SMTP.

Could you point out specifically which logs you're checking? We don't use SMTP on the Commons, so I want be sure we're looking at the right thing.

Where are you creating the "fake" entry? Where's the entry where the "regular" form lives?

The plug-in has its own logging that you can enable, but it discourages it enabling it for too long for performance reasons. It's in the plug-in settings:

Logging
Enable if you would like logging within Gravity Forms. Logging allows you to easily debug the inner workings of Gravity Forms to solve any possible issues.

I am entering fake data (entry) into the form. This particular form lives at https://khatchad.commons.gc.cuny.edu/wp-admin/admin.php?page=gf_edit_forms&id=5. The public access page to this form is at https://khatchad.commons.gc.cuny.edu/research/students/prospective-students.

Where's the entry where the "regular" form lives?

Entry just means data you put into the form (I entered fake data to test it). There is no "fake" form. The one and only form is accessible from the page https://khatchad.commons.gc.cuny.edu/research/students/prospective-students as mentioned above. Thanks!
