It appears to be a sproadic, server error.

If I hard refresh the URL to the slider image (or any direct image URL from the Media Library) several times:

Sometimes it loads, sometimes it doesn't.

We might want to involve IT in on this. It might be a server configuration problem.

Before I email IT, can you tell me if anything changed on the server recently? Like new plugin updates or anything like that?

I can't seem to duplicate the problem on the development server.

No new plugins, though I received a notice of an automatic update of WP earlier today:
