Description
While looking into #22255, I noticed that the 2FA team have made some changes to the display of the 2FA methods in the next version on the plugin.
Currently, we are on v0.11.0 and it displays as a three-column table with "Enabled", "Primary" and "Type" columns. The new layout will be more row-based. See:

I will need to check and see if anything needs to be done style-wise.
Boone, if I do not get to this in time for next week's maintenance release, can you omit updating the two-factor plugin to v0.12.0?
We would also need to update the help site with new screenshots.

I've finally got to updating our BuddyPress integration with the latest upstream two-factor plugin changes. For a list of code changes, see https://github.com/cuny-academic-commons/cac/compare/ea4c2dc779...f7d065cbe1.
I've attached a screenshot of what the new UI looks like. The UI is more streamlined and I'm now utilizing the same custom checkbox toggles as used on the "Account Settings > Notifications" page.
At the same time, I've also added support for the 2FA email token sendout to use our updated email design. See attached screenshot. Let me know if any changes need to be made to the email content.
This is scheduled for the next maintenance release on February 10th.
