Nest 'Change Avatar' page in 'Commons Profile' tab

Description
The ‘Change Avatar’ page is nested under the ‘Public Portfolio’ tab but still shows the ‘Commons Profile’ menu down the left side, with ‘Profile’ highlighted. Clicking ‘Public Portfolio’ tab takes user to the public view of the portfolio.
‘Change Avatar’ page should instead be nested under the ‘Commons Profile’ tab.

This is going to be sort of ugly to fix, so I'll do it. See #4207.

Fixing this alongside #4207 meant rethinking a couple of things. I think I've got it settled in https://github.com/cuny-academic-commons/cac/commit/820363f832f0bf39fc8f67c0eefc1d963cad9e49.
In order to retain nav access to the Change Profile Photo screen, I retained the profile tab but renamed it Photo. It's visible only to logged-in users viewing their own profile. I think this will actually have the effect of increasing the use of custom avatars on the Commons, since it's a touch more visible.
This will need some review during the testing period to make sure that avatar upload etc continues to work. For the time being, I'll leave this ticket open for feedback.

The page is a bit of a mess, which is why I cropped the screenshot. But the relevant part is the Photo item on the sidebar nav.
