At the CAT meeting today, a member came up to tell me that he had just had just received notice that a friendship request had been accepted. The problem is that he had no idea who the other person was, and he says that he did not make the request.

There is a small possibility that he somehow inadvertently hit "Add Friend," but that seems somewhat unlikely in this case -- the member was mystified by the occurrence and seemed to have no idea who the other person was.

I can communicate personal details through email; perhaps we can see when the FR was submitted?

I think that we should investigate this to the extent we can, but I don't know that we'll find anything. If that turns out to be the case, we can keep our eyes out for similar issues.
