Description
When you make a comment in a paragraph, read on, and then return after you have moved down the document, the comment icon just has a number, so it's not clear that you can add a comment by clicking on the number.

Very good point. I'd like to hold off on any changes here to see how frequently this comes up in user testing of the tool and what users would expect to see instead.
